My cash rate is billed at $75.00/15-minutes.

If North Coast Wellness is out-of-network we are able to provide a superbill that the patient submits to the insurance company for reimbursement.

We are able to accept credit cards, debit cards, FSA & HSA cards.

We do offer a sliding scale for those who need it - see FAQs for details. Payment arrangements must be made prior to appointments.

There is a $75.00 fee charged for completion of outside paperwork. Paperwork for FMLA will NOT be completed on an initial visit and is done at the discretion of the provider.

For paperwork that requires time over 15-minutes the cash rate is billed at a minimum of 30-minutes.

We DO NOT fill out disability paperwork for patients who have not been with the practice for at least 12-full months and it is 100% at the discretion of the provider if they will complete this at all.

Failure of charges to go through for services rendered will result in the next appointment not starting and/or your ability to schedule being held until payment of the balance or payment arrangements are made.

Missing visits can have a negative impact on your medication management and ensuring payments are current is the responsibility of the patient.

There is a $150 cancellation/missed visit fee applied for any visit not cancelled 24-hours in advance. This fee will be charged to the required credit/debit card of file.
